Passport Forms & Instructions

If you are applying for a new or replacement passport, or renewing a passport, you should read the Passport Application Form instructions carefully and fill out the form completely where indicated. Specific supporting documentation and application fees are required. Failure to provide the requested information on the application form may result in the denial of a passport, related document, or service. Important Notice: A new law (RPPL7-3) requires that by January 1, 2007, old passports will be replaced by new machine readable passports. The Embassy urges everyone to apply for a new machine readable passport in lieu of the old one.
